About the Event

  

LOCAL BOWLERS

1.  Form a Bowling Team of 4 people.  $25 Entry Fee per Bowler.  $100 Fundraising Goal per Bowler ($50 for age 17 & Under)

· Name your Team and Team Captain.

· Select your Bowling Session time (1:00-Session 1; 2:00-Session 2; 3:00-Session 4).

· Complete the registration form.  If you are not registering your entire team, encourage them to register as soon as possible so that your Bowling Session time will be secured.

· Print out the Pledge Form and start your Campaign [Minimum Goal per person: $100 ($50 for ages 17 & Under)].

· Be creative with your Campaign.  Consider posting on Social Media and make sure you are following us. Use #strikeituprenonv on all of your posts.

· Make a sign for your team no smaller than a standard poster board.  The sign will be displayed when you take your Team Photo and also near your Bowling Lane. The team with the most creative sign will WIN A PRIZE!

2.  On Event Day - Come dressed in your Inspirational or Breast Cancer Awareness Attire.  

· NO OUTSIDE FOOD OR DRINKS ARE ALLOWED.

· Registration opens at 11:45am FOR ALL SESSIONS. 

· You must be registered by 12:30pm for Session 1; 1:30pm for Session 2; and 2:30pm for Session 3.

· You will receive: a door prize ticket, shoe ticket, snack coupon, and Revivals Swag Gift of your choice.

· Raffle: Tickets will be available for gifts and 50/50. 1 for $1; 6 for $5; and 25 for $20.

· Bring your Pledge Form along with any Flat Donations in a sealed envelope that has your name, team name, and team captain name on the front.  All checks shall be made payable to Revivals Health & Wellness Council. 

· For Per Pin Pledges, you will need to contact your donors after your session and collect the money.  Submit all funds to Revivals by March 28, 2020 via check or money order to: PO Box 1495 Fernley, NV  89408.

· If your donors prefer to donate on line, refer them to the Donation link of our website. They will need to reference you in the memo section.

3. Event tickets ARE NOT REFUNDABLE.  Your purchase denotes that you agree to: 1) waive any claim of any kind whatsoever, whether resulting from an injury or otherwise; and to release, indemnify, and hold harmless, Revivals Health & Wellness Council and High Sierra Lanes (or the participating bowling alley), including but not limited to their respective directors, officers, employees, agents, consultants, volunteers, guests, and/or representatives from any and all liability including any loss, theft, or damage of materials, merchandise, equipment, or person occurring as a result of participation at this event; 2) be responsible for the health, safety, and welfare of any minors in your care; 3) notify us as soon as possible if you are unable to attend; 4) notify us by March 18th  if you want to transfer your ticket to another bowler; and 5) adhere to the event details herein.

OUT-OF-AREA BOWLERS

1. Form a Bowling Team of 4 people.  $25 Entry Fee per Bowler.  $100 Fundraising Goal per Bowler ($50 for age 17 & Under)

· Name your Team and Team Captain.

· Select a Bowling Alley in your area. Select a Bowling Time to bowl one game either on March 21st or March 22nd. Team Captain, notify Revivals Director via revivalshealth@att.net so that the game fee, shoe rental, and snack coupon can be generated. Revivals Swag Gifts will be mailed to the Team Captain. The Director will ask if your Team Captain can host a House 50-50 Raffle.

· Make a sign for your team no smaller than a standard poster board.  The sign is to be displayed when you take your Team Photo and also near your Bowling Lane. Of all the Out-of-Area Teams bowling, the team with the most creative sign will WIN A PRIZE!

· During your game, take pictures/video and post to social media. 

· SUGGESTED OPENING SCRIPT: I am Team Captain (state your name) of the (team name) from (city, state), and we are at (bowling alley) to Strike It Up For Breast Cancer Survivors.  I am here with my teammates (each member state their name).

· After bowling, send your Pledge Form along with a money order or check(s) to: PO Box 1495 Fernley, NV  89049 by March 28, 2020. At the discretion of the Team Captain, pledge forms can be sent individually or collectively as your Team.

· If your donors prefer to donate on line, refer them to the Donation link of our website.  They will need to reference you in the memo section.

HOW YOUR FLAT RATE AND PER PIN DONATIONS ARE DISTRIBUTED:

Proceeds from this fundraiser support the Annual Keeping the Faith Breast Cancer Survivors' Luncheon and our giving Campaign, Strong Arm Breast Cancer.  At the Luncheon we celebrate breast cancer survivors and honor those whose legacy inspires us. The event is named for Faith Fancher, who was a senior reporter with KTVU-TV in Oakland, CA. She allowed KTVU to report on her breast cancer fight not to bring attention to herself, but to encourage women to be vigilant, to be tested, and to be brave. Survivors and their supporters shop, dine, fellowship, and dance to celebrate one more year of victory; and rejoice about the memories of loved ones.

Strong Arm Breast Cancer is a giving campaign, to address Financial Toxicity. Financial Toxicity is the financial impact associated with the cost of breast cancer screening, treatment, and recovery which affects personal finances, career, and overall well-being.  We have found that this impact affects families throughout generations and therefore we honor our Legendary Ladies by supporting their families.
